Reporting Requirements and Deadlines for Fiscal Year 2012:

1. Signed Contract with Attachments -- Due November 10, 2011
   • Attachment A -- Scope of Services
   • Attachment B -- Scope of Services and Additional Terms
   • Attachment C -- Updated Budget
   • Attachment D -- Credit and Publicity Agreement
   • Terms and Conditions (PDF)
   • W-9 Form (PDF)
   • Contractor Authorized Signature Form (PDF) -- Required of all grantees
   • Authorization for Electronic Funds Payment
   • Credit and Publicity Kit -- Information about your grant's credit and publicity requirements, including an MCC credit logo.
2. Final Report -- Due 30 days after end of the project and no later than July 12, 2012.
