Reporting Requirements and Deadlines for Fiscal Year 2008:

1. Signed Contract with Attachments -- Due Feb. 29, 2008

• Agreement
• Terms and Conditions (PDF)

• Contractor Authorized Signature Form -- Required of all grantees
• Authorization for Electronic Funds Payment
• Credit and Publicity Kit -- Information about your grant's credit and publicity requirements, including an MCC credit logo.
2. Partner Form (PDF) -- Due anytime a collaborator changes.
3. Interim Report (Implementation Grants only) -- Due July 7, 2008.
4. Final Report -- Due at end of funding period.

Questions regarding a Local Cultural Council Program or PASS Program grant should be directed to the LCC Chair who approved the grant. LCC contact information.
