1. Signed Contract with Attachments -- Due February 10, 2012
• Attachment A -- Scope of Services
• Agreement
• Terms and Conditions (PDF)
• W-9 Form (PDF)
• Contractor Authorized Signature Form (PDF) -- Required of all grantees
• Authorization for Electronic Funds Payment
• Credit and Publicity Kit -- Information about your grant's credit and publicity requirements, including an MCC credit logo.
2. Interim Report - Implementation Grants -- July 6, 2012
3. Final Report - Planning Grants -- December 2012
4. Final Report - Implementation Grants -- December 2012
